Authorized applicant

Individual authorized by a control person of the company to open a Coinbase account on behalf of an institutional applicant

• Name
• Date of birth
• Address
• Proof of address (an official document dated within 3 months, such as a phone bill, utility bill, bank statement, paystub, tax document, etc.)
• Government-issued ID number:
US applicants: Social security number, permanent resident tax ID number, etc.
Non-US applicants: Driver's license number, passport number, residential country resident ID number, etc.
The number must be maintained and complete (not the last 4 digits, etc.)
• Government-issued ID (color copy of both sides)

Director(s)

Any individual responsible for the management of the company, including executive officers or senior managers.

Examples: Chief Executive Officer, Chief Financial Officer, Chief Operating Officer, Managing Member, General Partner, President, Vice President, or Treasurer

• Name
• Date of birth
• Country of residence
• Role within the company

Control person

An individual with significant responsibility to control, manage, or direct an institutional applicant, including an executive officer or senior manager

Examples: Chief Executive Officer, Chief Financial Officer, Chief Operating Officer, Managing Member, General Partner, President, Vice President, Treasurer, or any other individual who regularly performs similar functions

A document that shows a record of active directors elected by the entity’s shareholders

Source of Funds

A document that proves how this entity will initially fund this Coinbase account

• Bank, brokerage, or crypto statement dated within the previous three months

Certificate of Incorporation

A document that proves formation of this entity (typically issued by a local governmental agency)
